Nerve Blocks in Palliative Care

The authors rightly make the point that the successful use of interventional techniques first requires good patient assessment, and then a cooperative approach by palliative care and pain management teams, followed by clearly articulated plans for aftercare. Acute Pain, Vol 7 Karen Simpson and Fiona Hicks have put together their extensive collective experience to produce a clear and succinct text that manages both to set out the clinical problems and also to summarize the various techniques that are used currently ... This is a short and very readable book which I would recommend as essential reading for any medical or nursing profesional who is involved with the management of patients with cancer-related pain. My copy has sat easily in by brief case since it arrived through the post, and I have used it both for reference and as a teaching aid. It's been much more useful than those large, erudite tomes that are either gathering dust in my office or serving as a doorstop! British Journal of Anaesthesia .

New Concepts and Techniques in Pediatric Anesthesia: Vol. 20, No. 1 (March 2002) of Anesthesiology Clinics of North America

This book is an engaging volume that discusses practical concepts needed to take care of a child who is being anesthetized. The guest editors, along with 22 contributors, have done a wonderful job putting together a significant number of practice issues related to pediatric anesthesia in a comprehensive and clinically oriented pattern.
This monograph consists of 12 chapters, including one on cardiac arrest and another addressing ethical dilemmas in pediatric surgical patients. The contributors have done a nice job of writing their chapter with minimal duplications. The chapters are easy to read and easily understood. The material is presented lucidly with appropriate references and data.
The chapter on cardiac arrest reflects current practice and provides a quick algorithm for treatment of cardiac arrest in children. The chapter on psychological preparation of children prior to surgery imparts a good overview on the control of preoperative anxiety and the common clinical options available for anxiolysis prior to surgery in children. The chapter on the perioperative control of pain reviews current information regarding the use of cyclooxygenase-2 inhibitors, alpha 2 agonists, and opioids; the authors recommend using an integrated approach to achieve adequate analgesia. The chapter on pediatric regional anesthesia is well written and provides descriptions of the procedures and avoidable errors. There is even a chapter describing sedation and analgesia for pediatric patients outside the operating room or in the office. This is a good source for guidelines and policies issued by the ASA and JCAHO. This chapter also emphasizes the need for office-based anesthesia with its advantages and disadvantages.

Acute pain management: scientific evidence

This publication summarises the evidence currently available to assist health professionals in the management of acute pain. The field of acute pain medicine is a rapidly changing one.
Acute Pain Management: Scientific Evidence1 was first published by the National Health and Medical Research Council (NHMRC) of Australia in 1999. An editorial2 published the same year in the British Journal of Anaesthesia assessed the importance of this document along with Guidelines on the Use of NSAIDs in the Perioperative Period.3 It was noted that ‘there is still a need for properly conducted controlled clinical studies evaluating most areas of therapy in acute pain’ and that ‘guidelines can only remain valid for a relatively short period’ as the field of pain management is a rapidly evolving one.

The Aetiology of Deep Venous Thrombosis: A Critical, Historical and Epistemological Survey

What we now call deep venous thrombosis (DVT) has been elucidated by a diversity of investigative approaches during the past four centuries. The authors of this book survey the history of the field and ask: why has one of these perspectives the haematological/biochemical come to dominate research into the causation of DVT during the past 50 years and to exclude alternatives? In answering this question, they show that the current consensus model is conceptually flawed. Building on the work of William Harvey, John Hunter, Rudolf Virchow, Ludwig Aschoff and a number of pathologists in the mid-20th century, they offer a revised account of the aetiology of this condition. In the process they retrace and review the 160-year-old philosophical and methodological schism in biomedical research and, using DVT as an example, propose how this schism might be bridged to the benefit of both research and clinical practice.

Evoked Spinal Cord Potentials: An illustrated Guide to Physiology, Pharmocology, and Recording Techniques

Techniques using evoked spinal cord potentials (SCPs) have become important clinical tools for monitoring spinal cord surgery and diagnosing spinal cord diseases. This book surveys the neurophysiological and neuropharmacological bases of evoked SCPs with reference to animal studies and to recording those potentials mainly from the spinal epidural space. Generous use of illustrations promotes understanding of the neurophysiological and neuropharmacological backgrounds of monitoring spinal cord functions, and case studies provide additional insight into the monitoring and diagnosis of spinal cord dysfunction and disease. The book is especially useful for students and those new to the fields of clinical neurophysiology, neurosurgery, neurology, orthopedics, and neuroanesthesia. Modern Anesthetics (Handbook of Experimental Pharmacology) (Handbook of Experimental Pharmacology)

Some important constraints of anesthesia must be taken into consideration when the pharmacological properties of modern anesthetics are discussed. The most important of these could be that the target effect be achieved preferably within seconds, at most within a few minutes. Similarly, offset of drug action should be achieved within minutes rather hours. The target effects, such as unconsciousness, are potentially life-threatening, as are the side effects of modern anesthetics, such as respiratory and cardiovascular depression. Finally, the patient’s purposeful responses are not available to guide drug dosage, because, either the patient is unconscious, or more problematically, the patient is aware but unable to communicate pain because of neuromuscular blockade.

General ultrasound in the critically ill

General ultrasound in the critically ill describes a new clinical tool: ultrasound for the intensive care and emergency physician. Written by an intensivist familiar with ultrasound, it specifically details findings of immediate clinical relevance throughout its approx. 220 pages. Through a whole-body approach, this book considers new emergency applications regarding the abdomen, venous system, head, heart, and the most original topic, the lung. Flow charts are proposed to resolve daily intensive care and emergency occurrences: acute dyspnea, shock, unexplained fever, etc. The strong points and pitfalls of ultrasound are reviewed in detail. This book shows just how critical ultrasound has proven to be in satisfying a major concern in the intensive care and emergency medicine fields: speed and accuracy. With this ever-present requirement for rapid diagnosis in mind, General ultrasound in the critically ill provides a key to practicing a visual medicine, a great benefit to the critically ill patient, especially since ultrasound is noninvasive and can be done at the bedside. This volume is not only an exhaustive atlas dealing with the most variable aspects of the critically ill patient, but it is above all a guide, a permanent aid in the therapeutic decision

Pharmacology of Critically Ill (Fundamentals of Anaesthesia and Acute Medicine)

Most books dealing with pharmacology in intensive care focus on the drugs used to treat the illness or injury. This book addresses the other most important issue for those caring for the critically ill - how organ function will affect the drugs used, and how to adapt management accordingly. Contents include: general principles of pharmacology; absorption, transport, and elimination of drugs; modes of action drugs; renal failure; hepatic failure; cardiac failure; gut failure; brain failure; lung failure; and children safe prescribing habits. By discussing organ failure in all major body parts separately, as well as having authoritative chapters on the principles of drug interactions, "Pharmacology of the Critically Ill" provides a uniquely reference in this vitally important aspect of intensive care. Contributors from leading specialists around the world ensure that this book has international relevance.

This book deals with how changes in organ function can affect the way the body handles drugs. Most books on this subject deal with how drug treatments affect organ function. This book approaches it from the other way round. The information in this book will allow safer prescribing of drugs to critically ill patients. This is a clinical governance issue. Quality of care and quality of drug prescribing is vital to patient safety. With clinical governance and since the Audit Commission report on Intensive Care in the UK, this is even more spotlighted.

Preoperative Medical Consultation (The Medical Clinics of North America, Volume:87,Issue 1)

Preoperative medical consultation plays an important role in the practices of both primary care physicians and subspecialists. Despite this fact, many physicians feel inadequately trained to function as consultants in the perioperative period. Prior to 1980, there were essentially no textbooks on the subject, and there were only a few ‘‘landmark’’ papers. The November 1979 issue of the Medical Clinics of North America on ‘‘Medical Evaluation of the Preoperative Patient’’ was essentially the first ‘‘book’’ on the subject. Subsequently, numerous articles and textbooks on various aspects of preoperative medical consultation were published. Preoperative medical consultation was covered in the May 1987 issue, select topics in medical consultation in the March 1993 issue, and postoperative medical complications was the
topic of the September 2001 issue of the Medical Clinics of North America. The goal of this current issue is to review and update the major topics in preoperative medical consultation. This publication is not intended to be an all-inclusive reference book. It was written by practicing internists with extensive experience in perioperative medicine and expertise in their selected areas. The vast majority of authors are general internists and members of the Medical Consultation Interest Group of the Society for General Internal Medicine (www.sgim.org).Chosen
topics are the most important or those most commonly encountered in clinical practice. The 15 articles in this issue range from the role of the consultant, preoperative laboratory testing, and perioperative medication management to preoperative risk assessment of patients with diseases.
